Gold Package Consultation Details
KAN Management will complete all initial paperwork for new service :

Create Service forms as needed
Recruitment and training of staff infrastructure
MCO credentialing and Medicaid enrollment
Correspond with licensure reviews

PLATINUM PACKAGE

Payment Structure - 50% deposit due upon signing of contract: $7,250 A final payment of $7,250 is due upon the submission of Human Rights Policies & Procedures and the completion of all forms. Once the balance is paid in full, all documents will be shared with the provider via email and a binder with all printed documents.
CBS Platinum Package Consultation Details
KAN Management will complete all initial paperwork for the new service :

Complete all attachments for app/mod to include budget
Complete all policies & procedures mandated for service
Create HR forms
Create Service forms
Complete HR files for up to 3 potential staff (must have for walk through)
Complete dummy file (must have for walk through)
Correspond with licensure reviewer to ensure all paperwork and revisions are submitted timely
Write and create new service description
Human Rights Documentation Submission :
Complete Human Rights Policies & Procedure
Complete & Submit Compliance form

Correspond with Human Rights Advocate in reference to the policies
Implementation & Support :
Present at walk- through of facility
Conduct a strategy session with the company and assist them in developing a plan of action to implement new service within their organization. KAN will conduct a needs assessment & plan of action on company’s current infrastructure (i.e. Human Resource Department, Clinical Team, Systems Management, Billing, Programming/Services, etc).
Train owners and initial staff on service delivery, policies, forms, documentation.
Conduct one site visit per month, 3 hours for up to 3 months after obtaining license to assist with service implementation.
Create curriculum (Group homes, IOP, PSR programs only)
All Residential Group Home Program Fees
Seeking licensure, to include all services above ($25,000) project term 6 months.
Unlicensed residential services ($19,500) to include all services above project term 6 months.

Conduct two site visits per month, 3-5 hours for up to 3 months after to assist with service implementation.
Conduct one site visit for up to 3 hours for up to 3 months after obtaining license to assist with any loose ends.
Medicaid Enrollment & MCO Credentialing
Implement curriculum base service (group homes, recovery homes, IOP, PSR programs only)
Utilize KAN’s turn-key operations team to implement selective services.
Implementation & Support- $9,500.00 (flat rate-3 months)
Walk through of office/Licensure Review (Licensed Agency)
Conduct a strategy session with the company and assist them in developing a plan of action to implement new service within their organization. KÁN will conduct a needs assessment & plan of action on company’s current infrastructure (i.e. Human Resource Department, Clinical Team, Systems Management, Billing, Programming/Services, etc).
Conduct one site visit per month, 3-6 hours for 3 months to assist with service implementation.
Conduct one site visit each month, 2-4 hours for up to 3 months after the project is complete to assist with any loose ends.
